ABSTRACT

Abstract: Probiotic therapy is a viable alternative to chlorhexidine, a widely used antiseptic in dentistry that produces significant adverse effects. This systematic review aimed to analyze the effects of probiotics on experimental gingivitis in humans. Two independent reviewers conducted a comprehensive literature search until March 2019. Randomized clinical trials and controlled clinical trials were selected. Outcome data were extracted and critically analyzed. A total of five articles were included in the qualitative synthesis. No meta-analysis could be conducted due to the heterogeneity of the selected studies. The use of probiotics showed a slight improvement in clinical parameters. Changes in gingival crevicular fluid volume were lower in the presence of the probiotic than in the placebo group. All the studies showed that the immediate, positive effects of probiotics during the period of discontinued mechanical oral hygiene were due to the modulation of the host response, not the anti-plaque effect. Investigators should conduct randomized clinical trials to elucidate the mechanisms of probiotic action and develop improved delivery systems.

ABSTRACT

A gengivite é uma doença inflamatória que interfere nos tecidos de suporte dos dentes. Inicia-se pelo acúmulo da placa bacteriana sobre as faces dentárias e a margem gengival, promovendo a liberação de produtos tóxicos que são efeitos da combinação da bactéria e do hospedeiro. Há indícios de que os hormônios sexuais, principalmente os esteroidais, que estão presentes no ciclo menstrual, apresentem efeitos sobre a saúde periodontal, causando impactos diretos na inflamação gengival e em possíveis alterações de substâncias no fluido gengival. Para tanto, realizou-se uma pesquisa de campo com o intuito de validar a relação entre o ciclo menstrual e a gengivite e seus possíveis diagnósticos frente aos hormônios sexuais que se apresentam de forma tão relevante. OBJETIVO: O objetivo do presente trabalho é verificar através de uma pesquisa de campo, se durante o ciclo menstrual, as alterações hormonais causadas neste período têm capacidade de gerar alterações nos tecidos gengivais. METODOLOGIA: Trata-se de uma pesquisa de campo, realizada por meio de artigos indexados na base científica SciELO e avaliação clínica e sondagem da gengiva de 20 voluntárias durante o período pré-mentrual, menstrual e período pós-mentrual de cada uma.(AU)


Gingivitis is an inflammatory disease that affects the supporting tissues of the teeth. It begins by the accumulation of bacterial plaque on the dental faces and the gingival margin, promoting the release of toxic products that are effects of the combination of bacteria and the host. There are indications that the sex hormones, especially the steroidal ones, that are present in the menstrual cycle, have effects on the periodontal health, causing direct impacts on gingival inflammation and possible alterations of substances in the gingival fluid. Therefore, a field research was carried out with the purpose of validating the relationship between the menstrual cycle and gingivitis and its possible diagnoses against the sex hormones that present themselves in such a relevant way. OBJECTIVE: The objective of the present study is to verify through a field survey whether, during the menstrual cycle, the hormonal changes caused in this period have the capacity to generate alterations in the gingival tissues. METHODOLOGY: This is a field study, carried out by means of articles indexed in SciELO scientific basis and clinical evaluation and gingival probing of 20 volunteers during the pre-mentual period, menstrual period and post-mentual period of each.(AU)

ABSTRACT

RESUMO O fator mais importante na manutenção da saúde periodontal é o adequado controle do biofilme dentário. Face às dificuldades no controle mecânico por parte dos pacientes, métodos alternativos ou complementares têm sido utilizados como coadjuvantes químicos do controle do biofilme, auxiliando no tratamento da gengivite. Dentre esses métodos alternativos, destaca-se a fitoterapia, através da utilização de plantas medicinais. Dessa forma, esse estudo faz uma revisão sistemática tendo como objetivo a busca, na literatura atual, de estudos que comprovem a eficácia de produtos fitoterápicos à base de aroeira no controle do biofilme dental auxiliando no tratamento da gengivite. Foi realizada busca computadorizada nos indexadores MedLine, Lilacs, BBO e Scielo no período de Setembro a Outubro de 2014. Os descritores de pesquisa utilizados foram “biofilme”, “gengivite”, “aroeira”, “plantas medicinais” e “fitoterápicos”, bem como seus correspondentes em língua inglesa. Através dos descritores utilizados, foram encontrados 54 artigos. Após a aplicação dos critérios de inclusão e exclusão, chegou-se a 08 artigos: 05 revisões, 01 estudo clínico randomizado, 01 estudo experimental e 01 estudo observacional descritivo. De acordo com os estudos analisados, a aroeira apresentou atividades antifúngica e antimicrobiana eficazes, bem como potencial efeito de redução do índice de sangramento gengival, o que justifica seu uso no tratamento da gengivite.


ABSTRACT The most important factor to keep periodontal health is proper control of biofilm. Given the difficulties in the mechanical control by patients, alternative or complementary methods have been used as chemical supporting of biofilm control, helping the treatment of gingivitis. Among these alternatives, there is the phytotherapy, through the use of medicinal plants. Thus, this study it is a systematic review and aims to seek, in the current literature, researches proving the effectiveness of aroeira herbal products in the control of dental biofilm, assisting in the treatment of gingivitis. A computerized search was conducted in Medline, Lilacs, Scielo and BBO, from September to October of 2014. The search words used were “biofilm”, “gingivitis”, “aroeira”, “herbal medicines” and “phytotherapics” in Portuguese and English language. Through this method, 54 articles were found. After applying the inclusion and exclusion criteria, the research reached 08 items: 05 reviews, 01 randomized clinical trial, 01 experimental study and 01 descriptive observational study. According to the analyzed papers, the aroeira demonstrated effective antifungal and antimicrobial activities, as well as a potential reduction impact on gingival bleeding index, which would justify its use in the treatment of gingivitis.

ABSTRACT

A doença periodontal é uma entidade infecciosa que resulta da resposta imuno-inflamatória do hospedeiro aos microrganismos presentes no biofilme dentário, levando à destruição tecidual. O propósito do presente estudo foi avaliar a expressão imuno-histoquímica da ciclofilina A (CYPA), do indutor de metaloproteinases da matriz extracelular (EMMPRIN) e da metaloproteinase da matriz 7 (MMP-7) em espécimes humanos de gengiva clinicamente saudável (n=32), gengivite induzida por biofilme dentário (n=28) e periodontite crônica (n=30). Foram realizadas biópsias das três condições clínicas e feita a análise imuno-histoquímica através da contagem total do número de células positivas, correlacionando-a com parâmetros clínicos. A imunopositividade da CYPA, do EMMPRIN e da MMP-7 revelou diferença estatisticamente significativa entre os três grupos, com maiores percentuais de positividade nos espécimes de periodontite crônica, seguidos pelos espécimes de gengivite crônica e de gengiva saudável (p < 0,001). Foi evidenciada maior expressão de CYPA e MMP-7 nos grupos que tinham infiltrado inflamatório mais intenso. Foram observadas correlações das imunoexpressões de EMMPRIN, MMP-7 e CYPA, tanto entre si como com parâmetros clínicos (profundidade de sondagem e perda de inserção clínica). Foram verificadas correlações positivas entre a expressão de CYPA e as expressões da MMP-7 (r = 0,831; p < 0,001) e do EMMPRIN (r = 0,289; p = 0,006). Além disso, a profundidade de sondagem revelou correlação positiva, estatisticamente significativa, com as expressões de MMP-7 (r = 0,726; p < 0,001), EMMPRIN (r = 0,345; p = 0,001) e CYPA (r = 0,803; p < 0,001). Esses resultados evidenciam que a CYPA, o EMMPRIN e a MMP-7 podem estar associadas à patogênese e progressão da doença periodontal. (AU)


Periodontal disease is an infectious disease resulting from the immunoinflammatory response of the host to microorganisms present in the dental biofilm which causes tissue destruction. The objective of this study was to evaluate the immunohistochemical expression of cyclophilin A (CYPA), extracellular matrix metalloproteinase inducer (EMMPRIN) and matrix metalloproteinase 7 (MMP-7) in human specimens of clinically healthy gingiva (n=32), biofilm-induced gingivitis (n=28), and chronic periodontitis (n=30). Immunopositivity for CYPA, EMMPRIN and MMP-7 differed significantly between the three groups, with higher percentages of staining in chronic periodontitis specimens, followed by chronic gingivitis and healthy gingiva specimens (p < 0.001). Immunoexpression of CYPA and MMP-7 was higher in the intense inflammatory infiltrate observed mainly in cases of periodontitis. Analysis of possible correlations between the immunoexpression of EMMPRIN, MMP-7 and CYPA and between the expression of these proteins and clinical parameters (probing depth and clinical attachment loss) showed a positive correlation of CYPA expression with MMP-7 (r = 0.831; p < 0.001) and EMMPRIN (r = 0.289; p = 0.006). In addition, there was a significant positive correlation between probing depth and expression of MMP-7 (r = 0.726; p < 0.001), EMMPRIN (r = 0.345; p = 0.001), and CYPA (r = 0.803; p < 0.001). These results suggest that CYPA, EMMPRIN and MMP-7 are associated with the pathogenesis and progression of periodontal disease. (AU)

OBJECTIVES: Studies concerning side effects of chlorhexidine as related to the presence of plaque are scarce. The purpose of this study was to compare the side effects of 0.12 percent chlorhexidine gluconate (CHX) on previously plaque-free (control group) and plaque-covered surfaces (test group). METHODS: This study had a single-blind, randomized, split-mouth, 21 days-experimental gingivitis design, including 20 individuals who abandoned all mechanical plaque control methods during 25 days. After 4 days of plaque accumulation, the individuals had 2 randomized quadrants cleaned, remaining 2 quadrants with plaque-covered dental surfaces. On the fourth day, the individuals started with 0.12 percent CHX rinsing lasting for 21 days. Stain index intensity and extent as well as calculus formation were evaluated during the experimental period. RESULTS: Intergroup comparisons showed statistically higher (p<0.05) stain intensity and extent index as well as calculus formation over the study in test surfaces as compared to control surfaces. Thus, 26.19 percent of test surfaces presented calculus, whereas calculus was observed in 4.52 percent in control surfaces. CONCLUSIONS: The presence of plaque increased 0.12 percent CHX side effects. These results strengthen the necessity of biofilm disruption prior to the start of CHX mouthrinses in order to reduce side effects.

ABSTRACT

El 90 por ciento de los pacientes VIH-SIDA presentan lesiones bucales durante el curso de la enfermedad, observándose en la mayoría de los niños en las etapas iniciales, lesiones muchas veces no imputables al VIH. Determinar las manifestaciones bucales y su relación con el porcentaje de linfocitos CD4 y la carga viral, en 40 niños VIH/SIDA que acudieron a la consulta de infectología y odontología pediátrica de la Ciudad Hospitalaria “Dr. Enrique Tejera”. A través de un estudio descriptivo y de campo, se analizaron las variables, edad, género, categoría clínico-inmunológica, manifestaciones en tejidos blandos y duros, porcentaje de linfocitos CD4 y carga viral. El grupo de niños más afectado fue el de 1 a 6 años y el género femenino. La categoría clínica inmunológica más frecuente fue la B2 y la C3. En 65 por ciento hubo caries y en 80 por ciento lesión de tejidos blandos. La lesión bucal más común fue: adenopatías 57.5 por ciento, seguida de candidiasis 30 por ciento, y en igual proporción (17,5 por ciento) afta-úlceras y gingivitis; parotiditis y xerostomía 12.5 por ciento, herpes oral y petequias 10 por ciento, queilitis 7.5 por ciento, leucoplasia vellosa y eritema gingival lineal 2.5 por ciento. Independientemente del porcentaje de CD4 y carga viral, se evidenciaron manifestaciones bucales en tejidos duros y blandos, en una frecuencia relativamente alta, de allí la imperiosa necesidad de institucionalizar los programas de atención odontológica, así como la instrucción, sensibilización y motivación a los padres, representantes y personal de salud en el área pediátrica, sobre la importancia de la salud bucal en los niños VIH/SIDA.


90 percent of the childrens infected with VIH-AIDS may be at increased risk of experiencing oral lesions during the course of the illness, principally at the initial stage, the most of these lesions were not associated to HIV. To determine the oral manifestations and their relation with the percentage of lymphocytes CD4 and the viral load, in 40 children being treated for HIV-infection of the Paediatric Infection and Dentistry Service at the “Ciudad Hospitalaria Dr. Enrique Tejera.”. By means of a descriptive study of field, the variables age, gender, immunological clinical category, soft and hard tissue manifestations, percentage of lymphocytes CD4, and viral load were analyzed. The childrens more affected were the group of 1 to 6 years old and in feminine gender. The most frequent immunological clinical category was the B2 and the C3. In 65 percent there was dental caries, and in 80 percent soft oral lesion. The most common oral manifestation was adenopaty 57,5 percent, followed of candidacies 30 percent, and in equal proportion 17.5 percent aphthous ulcerations, and gingival erythema, parotid en largement and xerostomia 12,5 percent, herpes oral and petechiae 10 percent, cheilitis 7,5 percent, hairy leukoplakia and gingival erythema linear 2.5 percent. Independently of percentage CD4 and the viral load, oral manifestations on hard tissue were recorded as much as in soft tissue. The hard and soft weave injuries were relatively high, consequently, the urgent necessity to institutionalize preventive and therapeutic dental program, and also the education, sensibilization and motivation to parents, representatives and health personnel in the paediatric area; about the importance of oral health in HIV/AIDS children.

ABSTRACT

Periodontite e lúpus eritematoso sistêmico (LES) apresentam processos de destruição tecidual semelhantes. Pouca evidência é disponível sobre as possíveis relações entre as doenças periodontais e as formas de lúpus que afetam crianças e adolescentes, conhecida como lúpus eritematoso sistêmico juvenil (LESJ). Esta tese é baseada na hipótese de que adolescentes com LESJ podem apresentar condições periodontais diferenciadas devido a mecanismos inflamatórios e destrutivos alterados em comum. Dessa forma, os objetivos foram: (1) avaliar as condições clínicas periodontais de um grupo de adolescentes com LESJ e de um grupo de controles saudáveis sistemicamente; (2) verificar a atividade neutrofílica (elastase) e os níveis de IL-1 β e IL-18 no fluido crevicular gengival (FCG), bem como os níveis séricos de IL-18. A porcentagem média de placa visível, sangramento marginal e sítios com perda de inserção clínica ≥2 foi similar em ambos os grupos. A porcentagem média de sítios com profundidade de sondagem ≥3 mm foi significantemente maior nos controles que nos pacientes com LESJ (30% versus 16,9% - P=0,03). Os pacientes com LESJ ativos apresentaram maiores níveis de placa (P=0,01) e sangramento marginal (P=0,05) quando comparados aos inativos. Não foram encontradas diferenças na profundidade de sondagem e perda de inserção clínica entre os pacientes ativos e inativos. Quando comparados aos controles, os pacientes com LESJ tinham maiores níveis de elastase livre (427 versus 45 - P=0,03), mas níveis menores de Eα2MG (97,5 versus 295 -P=0,001), IL-1 β (13,4 versus 27 - P=0,05) e IL-18 (16,2 versus 41 - P=0,04). Na divisão por atividade só foram encontradas diferenças nos níveis de elastase, maiores no grupo ativo (601 versus 208,5 - P=0, 05). Os pacientes com LESJ apresentaram maiores níveis séricos de IL-18 (369,5 versus 331,19 - P=0,04) e VHS (20 versus 2,2 - P=0,03) quando comparados aos controles...


Periodontitis and systemic lupus erythematosus (SLE) share similar mechanisms of tissue destruction. Little is known about the possible association between periodontal diseases and the the forms of lupus that affect children and adolescents, i.e. juvenile systemic lupus erythematosus (JSLE). This thesis is based on the hypothesis that JSLE adolescents might have a different periodontal condition due to common altered inflammatory and destructive mechanisms. Thus, the aims were: (1) evaluate the periodontal conditions in adolescents with JSLE compared to those of a control group of healthy adolescents; (2) analyse markers of neutrophil activity (elastase), levels of IL-1 β and IL-18 in the gingival crevicular fluid as well as serum levels of IL-18. The mean percentages of visible plaque, marginal bleeding and sites with attachment loss (AL) ≥ 2 were similar in both groups. The mean percentage of sites with pocket depth (PD) ≥ 3 mm was significantIy higher in control than in JSLE patients (30% versus 16,9% - P=0,03). There were significant difference in plaque (P=0,01) and marginal bleeding (P=0,05) levels between JSLE patients with active and inactive disease. There were no significant differences in PD and AL in these subgroups. When compared to controls, JSLE patients showed higher levels of free elastase (427 versus 45 - P=0, 03), but lower levels of Eα2MG (97,5 versus 295 -P=0,001), IL1- β (13,4 versus 27 - P=0,05) and IL-18 (16,2 versus 41 - P=0,04). There were significant differences in the JSLE subgroups in the total amount of elastase, higher in the active subgroup (601 versus 208,5 - P=0,05). Se concluye que el control de la placa y los programas de mantenimiento son los factores fundamentales para la conservación de los tejidos remanentes, objetivo principal de toda restauración protésica; en tal sentido, también es importante considerar el control de la cobertura del tejido periodontal por alguna parte de la P.P.R. y la fuerza oculsal transmitida a las estructuras de soporte

ABSTRACT

La mucosa oral es frecuentemente asiento de lesiones vinculadas a enfermedades sistémicas, y dentro de éstas destacamos las discrasias sanguíneas. Se estudiaron en el Hospital de Clínicas José de San Martín, 15 pacientes con patologías de la mucosa oral cuyas enfermedades de base fueron leucemia, aplasia medular y linfoma. De estos, 10 presentaron manifestaciones hemorragíparas, 7 pacientes con lesiones ulceronecróticas y 2 casos de hipertrofia gingival por infiltración neoplásica

Los estudios serologicos para inmunofluorescencia indirecta (IFD) como las bipsias de la piel para inmunofluorescencia directa (IFD) en las enfermedades vesicampollosas nos pueden dar una informacion diagnostica. Se describen las principales enfermedades ampollosas, como dermatitis herperiforme, penfigos, penfigoide, herpes gestationis, dermatits ampollosa de IgA lineal, etc., haciendo enfasis en la clase, caracteristicas y localizacion de los diferentes anticuerpos a nivel de la piel de estas entidades, como tambien la presencia de antigenos de histocompatibilidad. Se menciona como la toma de la biopsia de piel y mucosas difere para cada entidad para lograr una mayor positividad
